A family owned and operated nursing home that specializes in behavioral health.
Located in Peninsula, just on the edge of the Cuyahoga National Park.
We have a 95 bed locked nursing home as well as a 10 bed group home on the campus.
Job Summary Looking for positive and reliable Licensed Social Worker.
Experience with Mental Health and behavioral residents preferred.
Full-time; Monday-Friday position.

Experience with completing the MDS and care plans preferred.

Qualified candidates will be contacted for interview.
Identify medically related social and emotional needs of each resident.
Develop preliminary and comprehensive assessments of the social service needs of each resident and complete the social services sections of the MDS in a timely manner.
Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
Provide information to resident/guardian as to Medicare/Medicaid, and other financial programs available to the resident.
Provide consultation to members of our staff, community agencies, etc, in efforts to solve the needs and problems of the resident.
Refer resident/guardians to appropriate social service agencies when the facility does not provide the services or meet the needs of the resident.
Review complaints and grievances, work with departments to make applicable changes, and provide follow up.
Perform administrative requirements that are required by the Administrator, Company and/or State/Federal Government.
Participate in discharge planning, development and implementation of the social care plans and resident assessments.
Participate in facility surveys made by authorized government agencies.
Provide all residents with information concerning resident rights, living wills, how to file a complaint or grievance, etc.
Actively participate in multidisciplinary meetings such as QAPI, Care Conferences, All Staff, etc.
Instills confidence in the resident/guardians by performing all job duties in a competent and caring manner.
Develop, participate, and implement training programs and in-services for the staff; including PMT training.
Maintain healthy and professional boundaries with residents.
Maintain excellent attendance and punctuality.
Perform other duties that may be assigned.
Specific Requirements:
Maintain the confidentiality of all resident care information and any other personal information about the resident.
Basic computer proficiencies are required.
Examples are:
ability to send and manage incoming emails; ability to turn computer on/off and operate a mouse; comfortable with opening, making changes to and saving documents.
Must be able to read, write, speak and understand the English language.
Must possess the ability to develop good working report and deal tactfully with other departments, staff, residents, guardians, family members, visitors, government agencies/personnel and the general public.
Must be able to effectively communicate information concerning a resident(s)/family to the appropriate person(s).
Must be willing to accept and seek out new methods and principles and be able to incorporate them into daily work flow.
Must use safe work practices by following policies and procedures.
Must maintain a neat appearance and follow the required dress code policy for your department.
Must have patience, tact and a positive attitude, as well as the willingness to handle difficult residents/guardians.
Accepts reassignments of duties and/or case load as needed to provide the necessary care for all residents.
Experience:
Social worker experience working in a long-term care, home health or hospital setting is required.
Working with the mental health population is strongly desired.
Experience working in a supervisor capacity in a healthcare related facility is strongly desired.
Education:
Must possess a Bachelor's Degree, preferably in Social Work or a healthcare related subject.
Master's Degree is preferred.
Must be licensed in Social Work.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
